| Abstract: | (from the chapter) This commentary is in response to an article on pharmacological treatment of major depressive disorder by Papakostas, Bech, and Fava (see record 2010-04828-008). The chapter presents a very good review of the current state of the pharmacological interventions in terms of efficacy, safety and tolerability of antidepressants for the treatment of major depressive disorder. The review is valuable for the practitioner as it covers all the groups of drugs, adopting a comprehensive and critical approach. There are a number of conclusions that are of special value to psychiatrists working in developing countries. From the perspective of developing countries, there are three specifically important issues in the pharmacological treatment of major depressive disorders. Firstly, almost all people with major depressive disorder will be treated at the level of primary health care due to the extremely limited number of psychiatrists. Secondly, given the lack of major breakthroughs in the pharmacological treatment of major depressive disorders, there is a need to examine the effectiveness and acceptability of nonpharmacological interventions. Thirdly, the examination of the indigenous methods of treatment (e.g. yoga, meditation, focusing) is an important opportunity for professionals working in developing countries.
