| Abstract: | (from the chapter) Lifetime rates of anxiety disorders in the HIV-positive population are similar to those in the population without HIV infection, but data suggest that the patients with HIV disease have an increased rate of generalized anxiety disorders and current (as opposed to lifetime) anxiety disorders. It is important that providers recognize anxiety disorders in those with HIV because treatment can improve quality of life, decrease disease transmission, and improve health outcomes. Generally the treatments that are used for other anxiety disorders are appropriate for those who have HIV disease; however, providers need to be aware of the risks of drug interactions between antianxiety and anti-HIV medications. Some HIV-positive patients may also be at greater risk for cognitive effects or substance abuse than the general population. Psychotropic regimens should always be thought of in the context of the antiviral regimen, the stage of HIV disease, and any alternative medications the patient might be taking. In particular, providers should include in their differential diagnoses of anxiety disorders other illnesses that can mimic them, including depression, substance abuse, minor cognitive motor disorder, HIV dementia, and delirium. Behavioral treatments can be a powerful and important aspect of managing anxiety in HIV disease. In addition to decreasing or helping to manage anxiety symptoms, they can provide insight and support for the patient and his or her family system. Alternative treatments such as meditation or visualization should be embraced by the provider, and special interventions are available for those triply diagnosed with HIV, anxiety, and substance abuse.
